Specify which region to create pre-paid GitHost instance
When a request comes in to create a new pre-paid GitHost instance, the support team has all the information we need to create it, except which region to create it in.
@rdavila is there a way we can get this to be required for submitting the form? It would be even better if there could be an option to notify that they want us to change the billing on an existing instance to pre-paid.

@dewetblomerus we use all the info that is provided by Zuora to generate the notification email so looks like we may need some custom fields in Salesforce and Zuora in order to accomplish this. /cc @francisaquino
@dewetblomerus It may be best to handle this as a macro. We also need to know the subdomain, or custom domain that the customer wishes to use. We can create a macro that asks all the necessary information. How does this sound?
@dblessing a macro might be a better boring-solution because of our team having direct control over it so we can make changes in the future. I am not a Zendesk admin so I can't create shared Macros, would you do it?
