Issue/MR page header inconsistency

Moving my comments from https://gitlab.com/gitlab-org/gitlab-ce/merge_requests/2826#note_3788649 to an own issue as no one seems interested there.

!2826 (merged) changed the "header line" of issue pages to, for example:
Issue #13359 · by Jeroen van Baarsen · 6 days ago, i.e. changing opened by to by.

For merge requests, we still have:
Merge Request #2826 · opened by Alfredo Sumaran · 2 days ago which is terribly inconsistent.

And once again: I think the "opened by" is important to have as it also gives the "date" after it some context. Otherwise the 6 days ago is a bit weird. Was that the creation date? Edit date? Last comment date?

I think a lot of GitLab's awesomeness is due to attention to details like these. And please keep that up!

@jschatz1 one more thing, MR should start with ! not #. Also please get rid of • and keep it like a solid statement.

@pbr I understand what you mean, and it makes sense. Before due to edit, and comments mentioning in header it was too much. Now it's gone, so maybe Issue #13359 opened 6 days ago by Jeroen van Barren make sense. @dzaporozhets @jschatz1 what do you say?
