Add fog-azure

Hello, it would be great if you would add fog-azure in a feature release. Currently I'm running on azure so it would be a great addition.

We'll be removing fog and therefore all its 39 dependencies. Please comment again if you still want this explicitly included.

@connorshea I think it makes sense to support AWS, Google Cloud, and Azure, not just AWS alone.
