Form alignment in Mattermost notifications form is off

Form should be aligned with top box:

assigned to @samrose3

removed assignee

@dimitrieh can you confirm this is a bug? The form seems indented one extra level

@filipa doesn't seem as a bug to me

although i do say it isn't that aesthetically pleasing. This will however be handled in the future with https://gitlab.com/gitlab-org/gitlab-ce/issues/28451
