More consistent padding of notes, system notes, and note form

- "Added N commits" system note has too much bottom padding
- Form avatar and textarea are not aligned with note avatar and text
- Form textarea has more top padding than necessary
- Padding of regular note in discussion and regular note on root is inconsistent
- Padding of system note in discussion and system note on root is inconsistent
- In discussion, system note icon is not horizontally aligned with regular note text
