Added a template for database changes
This adds a new merge request template to be used when making database changes. It has a rather long checklist, but most of it is fairly easy to follow (and some of the tasks can be checked off at the same time).
One thing I'm wondering is how we can make it clear to use specific templates; it might be easy to overlook the dropdown and just use the default MR template instead.
