Added user avatar to header

What does this MR do?

Adds the users avatar to the header with a dropdown for profile & sign out.

@iamphill let's remove from sidebar avatar in a separate MR.

@iamphill You have 9px (or 10px if you count the border, but I guess we wouldn't) padding on the top (above profile) and the bottom (below sign out) and 8px in the middle. Can we make it 8px on top bottom and in the middle? To my eye it's a little too much space on the bottom. Should be equal. BTW when I say border I do not mean the dark outside border. There is a slight shadow in there that I am talking about. Obviously wouldn't count the dark gray border.

@iamphill once this merged - please create MR to remove user section from sidebar. It makes no sense render user avatar in 2 places on the page.
