Separate prometheus

Run Prometheus in own container. Prometheus is configured to monitor any service annotated for monitoring, and will also monitor nodes (cAdvisor).

Depends on !6 (closed).

I would say this includes !6 (closed) as opposed to depends on it. I would like to see the port name be a full prometheus if not gitlab-prometheus in place of gitlab-prom, for clarity.

@WarheadsSE Thanks for the comments. Can you take another look? We'd like to use the version with Prometheus for the webcast today (Wednesday).

@joshlambert Will do today. If we get it all sorted, we should try to get the sales demo to include it somehow.

For now, we'll separate after we have it in, moving the separate to the 8.17 change set.

This actually does not close #2 (closed) as of this time.
