Favicon, README fix, LICENSE

• Adds a Favicon
• Fixes the README rendering incorrectly
• Adds a license (Same as is used in GitLab CE, but with 2015-2016 for copyright date)

I'm not sure how pushing the Status Page to production works, but would you (or whoever is responsible for that) mind pinging me when it happens just to make sure that I'm around to fix it should anything break? I tested it manually in production on my local machine, but the Asset Pipeline can be finicky.
