doc: package main can be directory with an index

Rodrigo Muino Tomonari requested to merge github/fork/bmeck/package-main-doc into master

This behavior dates back to 2011 but was not documented.

@isaacs made the original change.

I am unsure if package.json in a directory should also be supported here and if ESM behavior should also have this.
