meta: replace feature request template with form

The bug report form doesn't seem to result in many invalid/spam reports, but the template still results in lots of issues opened by apparent bots that don't modify the default template imput. Change the feature request template to a form to hopefully better serve people proposing features and reduce bot-generated junk issues.
