tools: simplify `fileURL` test fixture → `fileHref`

All uses of this test utility merely converted the output to a string via inconsistent ways (often coercion, which is confusing).

A couple quasi-unrelated hits were picked up by my replace-all, but they are actually the same kind of thing, so I included them for consistency.
