Propose v6.2.1

Rodrigo Muino Tomonari requested to merge propose-v6.2.1 into v6.x

New plan is to do an RC with https://github.com/nodejs/node/pull/6895 this week, and continue to work on that pull request's test before doing a release next week.

Proposing a patch this week. There are two minor commits but they are low priority. Above all, I'd like to get https://github.com/nodejs/node/pull/6895 in (The OS X stdio fix), so this may be delayed a day or two.
