Prey can't be regurgitated when size difference is too great
When a large pred regurgitates a smaller prey, the prey will be automatically swallowed back and can't get out.
This is likely caused by the exponential increase of the swallow progress, which makes it so that the more swallow progress you have the faster your swallow progress further increases... mixed with micro / macro size difference further adding to the progress, as well as prey being given 0.5 swallow progress when being regurgitated. As such this is normal with the current mathematics, and will need a different means of prevention.

Found a potential solution. Unless a better suggestion is presented, I will likely attempt this fix.
When prey is regurgitated, they are currently given the spawn shield. We could make it so that players with the shield active can't be swallowed (may have swallow progress but not end up in the stomach), as is already the case with health and armor limits. If the shield lasts for at least least a second, the prey may have enough time to distance themselves from their predator, enough to make the swallow progress decrease rather than increase with time which immediately sent them back in.
