doc,lib,test: rename node-report to report

Rodrigo Muino Tomonari requested to merge github/fork/cjihrig/suite into master

This PR renames "node-report" to "report" throughout the codebase. The motivation is to begin making the distinction stronger between core's diagnostic reporting and the original node-report module on npm.
